SOUTH BEND — Gerald Bernard “Jerry” Chachulski, 76, of Grand Rapids, Mich., passed away Friday, January 18, 2013.

Born in South Bend, he is survived by his beloved wife of 53 years, Beverly (Carlson); daughters, Leigh Ann and Caryn (Matt Cowall); granddaughter, Margaret; and sisters, Patricia Skudlarek and Diane Hahaj of South Bend.
